Faculty Development Workshop Basic Certificate Application Form

Successful completion of four eligible workshops over a two year period is required for the CTL Faculty Development Workshop Basic Certificate. Workshops completed prior to Fall 2004 will not be applied towards the Basic Workshop Certificate.

Workshops that meet the requirements for the CTL Basic Workshop Certificate follow

There are four required workshops for the Basic Certificate. Please supply the workshop facilitator and the beginning workshop date (if you do not know the exact date you may approximate) or the semester and year for each workshop that you have successfully completed.
