Faculty Development Workshop Basic Certificate Application Form

Successful completion of four eligible workshops over a two year period is required for the CTL Faculty Development Workshop Basic Certificate. Workshops completed prior to Fall 2004 will not be applied towards the Basic Workshop Certificate.

If you would like to apply for a CTL Faculty Development Workshop Basic Certificate, please complete this form. If you have any questions, call the Center for Teaching and Learning (CTL) office at 240-582-2770 or send e-mail to ctla@polaris.umuc.edu.
