Office of Accessibility Services: How do I register for disability support services?

You must be actively enrolled as a UMUC student to register for disability services. Prospective students are welcome to contact the department for information prior to enrolling at UMUC.

The first time you register for services, you will be required to provide current (within three years) paperwork documenting your disability to support your request for accommodation. The following link contains forms needed by your provider (licensed professional) to verify and explain the nature of your disability: Disability Verification and Accommodation Request Forms.

You'll also need to complete a Student Intake Form, which can be accessed via the link above.

You'll need to have your provider fax your completed documentation to Disability Services at 240-684-2590, or mail the documentation to:

The Office of Accessibility Services will conduct a preliminary review of your registration form, documentation, and submitted records and materials to determine if appropriate and complete. Our office will inform you if clarification and/or additional documentation, records and/or materials are needed.
