Title Page

The title page should have the following information appropriately spaced and centered on the page. The title sheet should not have a page number, although it should be considered as page “i.”

The table of contents should have a heading, and each section and subsection within the report should be indicated by the page number with which it begins. The table of contents should be numbered page “ii” (or “iii” if an acknowledgement page is included). The appendices section should specify each appendix with identification name and page number. Please note the page numbers listed below are relative, and the approval sheet goes on top of the title page
