Use of Human Subjects Guarantee of Compliance Form (Printable Form)


 The following language must be included verbatim in any survey or research instrument involving human subjects and being conducted as part of a UMUC course or project:

“This research is being conducted in fulfillment of requirements for a course or project at University of Maryland University College (UMUC). It is, however, not sponsored by UMUC. The research topic and research instruments have been determined by the student researcher, in conjunction with his or her faculty advisor.


Please complete the following question(s) and sign/date the form.
  1. Does this project involve the collection of information from human subjects by means of tests, surveys, interviews, or observations?
    Yes (Answer question #2 and question #3)
    No (Answer question #2, sign/date this form, and submit it to your advisor)
  2. Does the research activity and data deal with sensitive topics such as illegal conduct, drug use, sexual behavior, or use of alcohol?

  3. If the project does involve human subjects, is the nature of the data collection activity equivalent to any of the situations described in the box below? (If yes, check the relevant activity or activities below. This exempts the project from review by the Institutional Review Board.)
