Financial Aid Forms
Additional documentation must often be submitted in order to complete the financial aid application process. If requested, here is a listing of available forms through your MyUMUC student portal or PDF format (provided below). All of the PDF documents can be filled out electronically, printed, signed and sent to our office. Please pay close attention to the correct aid year when submitting these documents.
Available through your MyUMUC student portal. Scholarship applications are only available for the current 2011-12 year:

Note: To access documents through your MyUMUC student portal, please take the following actions:
- Click on myFinances > Financial Aid > View My Financial Aid > and Click on the aid year
- If you have met the criteria for a Scholarship and have been notified via checklist, you will see the link to apply for Scholarships after selecting the aid year.
- Click on Required Documents to see a listing of documents being requested by our office
