Part 2: Workplace Learning Activities |

Below, list the specific tasks to be completed during the 15-week session and indicate the number of hours per week or the percentage of your time you will spend on each of the activities over the 15-week session. Also describe the resources that will be needed to complete each task, such as training, manuals, software, special equipment, team or supervisor input, etc.
Note: To earn 3 workplace learning credits, you must work at least 12 hours per week for 15 weeks and list four specific new tasks you must complete in the new position or project(s). To earn 6 workplace learning credits, you must work at least 20 hours per week for 15 weeks and list five to eight specific new tasks you must complete in the new position or project(s). |

Part 3: Knowledge and Skills to be Gained from Your Workplace Learning Experience |

| This list will help you identify additional courses that you might want to take in future semesters to advance your career. Note that workplace learning credit will not replace any course offered for credit at UMUC. Workplace learning credit may be applied toward your major or minor (up to 6 credits), toward your related courses or as a general elective. Your counselor or workplace learning staff will explain how you can apply your workplace learning credit to your degree requirements at UMUC.

Review your learning proposal with your supervisor at work. Ensure your supervisor understands and supports your participation in this workplace learning experience. UMUC's workplace learning staff is available by phone at 240-582-2280 to provide additional information to your supervisor, if needed.
Your supervisor's input may also help you describe and prioritize your new tasks accurately. Note that if your learning will continue beyond this time, you may be able to earn additional workplace learning credit after completion of this session.
When you submit this form, you will receive a copy via e-mail for your records. Because your learning proposal will be used as the basis for your first assignment after enrollment, it is important to save this copy. |
